What is Professional Geriatric Care Management?
Geriatric Care Management professionals help older adults and their families develop solutions to the challenges of aging. They are expert problem solvers, skilled at providing options and making recommendations to support a client and their family in making informed decisions often times functioning as an advocate, liaison, and/or support system for them in a time of need.
Geriatric Care Managers are trained healthcare professionals qualified to assess the biological, psychological and social needs of older adults. They formulate care plans and recommend appropriate resources and support to older adults and their families. Ongoing screening, monitoring and follow up are provided to closely address any changes in the older adult’s daily life.
